Please use and reference the forms attached below when signing up for a school sports team. PLEASE READ EACH FORM. You must present the CCSD • Athletic Registration Form and the Interval Health History Form to the school nurse to obtain your clearance form known as the “Green Card” to be eligible to participate. The school coach can only permit you to play if you have obtained the clearance form known as the “Green Card”. Sports physicals are valid for a period of 1-year plus the remaining days through the end of the month of its expiration. Physicals that expire during the season are valid through the end of the sports season so long as the Interval Health History Form was completed and approved by school personnel.
*It is strongly recommended that parents and/or students meet the nurse in person to make sure all paperwork has been properly completed as opposed to mailing forms. Forms filled out incompletely or with errors will result in a delayed Green Card approval, which could result in your child missing out on a first day of practice.

Notes: Please make sure that the head coach for the team knows a student is interested in participating. Contact information for the coach can be found on our directories page by clicking here. Once your Green Card is secured, the student should hand the Green Card to the coach at the first practice or other time designated by the coach. If the school nurse keeps the card, the coach will be collecting the cards from the nurse before the first practice.
